lost a phone with a public mobile sim.

Hi people, We lost a phone with a public mobile sim card.  I had not set up the self serve account on that phone.  How can that one be cancelled and a new one issued?  I have all the paper work. Thanks.

Login disabled

I need to log in to my self serve but I get an error that login has been disabled. It's not giving me the option to change anything. Here's the error message: ForgotPassword/LoginUnsuccessfulUser_LoginDisabled I need to make a payment asap.
