Can't verify login

I created my account and my phone is active but I don't get a verification code by text and there is no option for an email code.  When I try to reset my account password it says it doesn't recognize my email or phone number.

I had some one from Telus call me and new old some of my information. They offer me the same plan for $10 less then I am currently pay and the they wanted my driving Licence and SIN number or credit card. Is this a scam.
