#21# disable call forwarding stopped working

As of yesterday, my calls keep getting forwarded even after sending #21#.  Opening the call forwarding screen that becomes available after doing a *21* and selecting Disable also doesn't work.  Galaxy S6.  How can I disable call forwarding?
