For the visa application, the phone bill is required to show the name, phone number, and billing date. However, the current downloaded bill does not include the phone number or billing period. What should I do?

As mentioned, Public mobile is a prepaid carrier and doesn't do a credit check or identification check when customers sign up with service. . Any customer can use any name and address, since PM does not verify.
Its invoice should not be used for any formal identification process like Visa application.

sorry, PM is a prepaid provider and usually visa or government won't take PM invoice as aaddress proof.
And I don't think PM agent can alter the format of the bill either. But you can check .
