Where can I get a call log that has my name and number on it?

I need to submit my call log for ICBC residency verification. The data usage log I downloaded is an Excel file that doesn't show my name or account information. Where can I get a report that shows my name, phone number, and the activities on this number for the past three months?

Why would you supply ICBC with your personal information. 3 months of activity...thats BS and NONE of their business.  

You can create your own invoice though with your name and address and your PM account #. 

If that is what they will accept.....give it to them.

Your phone number might be visible on call display but not a name. PM does not provide that info.

your Public Mobile account can't be used to verify residency as none of your info in your account (name/address) is verified by Public Mobile.
