Login to your account online or on the PM app and click on lost/stolen phone to stop service.
Once you get a new phone and SIM, you will need to enter the new SIM card number.

If you lost your phone and SIM, you would need New SIM, new phone and then log in to your PM account and transfer number/service to new SIM.

Just so we understand since we are all customers here. You lost your phone and you want to report it lost? Or you lost your phone, reported it lost and have a new SIM card you want to register?
