Locked out of my account, and need PUK Code for sim card

I forgot my password and security question for my log in information and have been locked out. My Sim Card wont work unless i put in the PUK Code required into my cellular device. Looking for support on both matters

Please go to https://widget.telus.tiia.ai/publicmobile/publicmobile.html and type "forgot security questions answer" to open a ticket to a CSA. While talking to the CSA, you can also discuss the PUK issue.
