HI @Mike5508-11 the PUK code is either a lock to your phone or your SIM card
have you been using PIN on your SIM (like every reboot of the phone require to enter PIN )?
Or you have changed a new phone?
What brand is your phone? Samsung?
What was the exact message for the PUK? It would have to determine what kind of PUK you need

If you have not recieved a reply from a CSA after contacting customer support either by submitting a support ticket via Simple--Simon or by sending a orivate message you will have to contact them again. Are you sure you need a PUK code? What is the make and model of your phone? Did you recently activate?
If you are positive it is the PUK code you need please contact customer support and keep an eye on the envelope icon at the top right corner of your screen next to your avatar for a little number to pop up indicating a reply from a CSA. Responding promptly will speed up service times but expect a reply within an hour.

- SIM network PIN blocked. Enter SIM network PUK." is asking for the phone’s (often Samsung) “master code" after entering the wrong code too many times. Samsung and/or your previous carrier can give you this.
- SIM network unlock PIN" is asking for your previous carrier's unlock code - just contact them with your IMEI (dial *#06# to get it) and they'll give it to you. If you enter this incorrectly too many times, you'll get the error message above.
- PIN unlock key (PUK) is a SIM-specific code used to prevent unauthorised use of a SIM card.
The PIN (Personal Identification Number) enables secure access to the SIM card and network. It is provided with your SIM card.
If the PIN code is entered incorrectly 3 times, the phone is locked (except for emergency calls) and requires a PUK (Personal Unlock Key) to reactivate it.
