For the International LD Bonus, where can I find the Country List?

Which country are you trying to call?  Since PM does not have any extra charges, you can always try the international phone number.  If the call goes through, then you will know that your add on works for this country.  If it fails, then the country is not included in your add on.  No extra charges.
