Have you ever created a Self Serve account before?
If not, you can register for one here: https://selfserve.publicmobile.ca/self-registration/
If you have registered for an account previously, and your account has not been suspended in non-payment status for over 90 days (if so, your account is closed and phone number is lost); and your Forgot your Password is not working, contact CSA to help you get back in to your Self Self account.
