When does a referral receive their $10 off?

Is the $10 off immediately when they sign up, or is it a credit on their account that is applied the next month?

Usually you will see the credit, on the next day.

the $10 is a credit on the account for the one who joined PM using the code.  The $10 will be added to his/her My Account within 72 hours of activation.  Since it it s a credit, the money will be used towards next renewal or add-on purchases
