Incoming calls logged in call history as UNKNOWN

I have the same issue. Tried full factory reset, even tried custom rom to fix it. I'm thinking must be a pm issue

Check usage logs in the self serve portal.  If dialer logs differ, problem is definitely with phone.  It can also be at the caller end where they may have invoked some sort of privacy setting.  Again that would appear in the usage logs in the self serve portal.

@aaronmartin As said earlier in the thread it's something in your phone settings. Is there a privacy setting that manages that in your phone?
