I did read what the OP asked for. That bill can be used to submit to an employer for expenses but the OP asking for name and address usually means they want to bill to renew driver's license and want to use it as proof of address. The bill can't be use for that.

Login your self service account using incognito or private mode by using laptop not mobile , then in payment history, you will find your payment history and download it. It will show your full name and your physical address and your account number.

That bill can't be used to prove you live there. Public Mobile will put any address and the ID places know that.

If you login to your account and go to Profile>Account Information…there you will find name, address, your Alternate Phone Number and Your Current Phone Number (cell).

Go into your My Account. Download a PDF of 30 days of your transactions. You will get exactly what you are asking for, as long as you were able to input your city.

Yes, PM agent can prepare you the invoices., but it might need a bit of time, couple weeks.
Just submit a request at : https://publicmobile.ca/chatbot.
First type Contact,
then select the only choice there: "Contact..",
from the list of , choose "Other"
finally click link the blue link about submit a ticket
Another page will open and just follow: .
After ticket is submitted, make sure to check your Community Inbox(top right corner envelope icon) periodically for response from PM
If you have problems submitting a ticket, you can open ticket by private message (but this can take longer):

If you need an invoice, you'll need to contact the CS Agent, you can't generate that yourself.
You can contact an agent using the chatbot. Type "agent" and click contact us to go through the wizard.

If for some reason they don't respond to the ticket, you can try to send an email directly to the CS Agent here.
