I know of someone who had a simillar issue and that root cause of it was a defective sim card.
Try a new sim card.

You already did this? Settings --> General --> Reset --> Reset Network Settings
Did you reboot?
If that doesn't work then perhaps a hard reset might be in order.

Try a hard reset.... (Might be an easy fix). Worked for some.
With the new operating systems (ios), a lot of those settings aren't accessible anymore.
