This same answer was already given. However, since this question was asked more than 4 hours before your reply, any such lockout of the sim card number would no longer be a issue.

try dont go back after you submitted.. if you did so, please wait 50 min and try again from the first step . it should working.

@Shannon1101 That's an activation issue. You need to wait for an hour and try again with cleared cache and cookies on your browser.

@Shannon1101 As long as your credit card has not been charged try again in an hour or two. Incognito mode may help as well.
PM's activation site cannot go backwards.
Speaking of credit cards - do not enter anything in the "unit #" field nor a space in your postal code.

@Shannon1101 the sim is probably locked. Wait an hour from last attempt, clear your browser history, go to private/ incognito mode and try again to activate your SIM card. Welcome to PM,.Stay safe.

Try using a different browser and incognito mode. Also clear your cache and cookies then reboot.
