Hi,
I have activted a new sim, but when inserting it into an unlocked phone, it is asking me for a PUK code. I have messaged the mod team, but haven't got a response, I'm already paying for service that I can't use!
I have a second sim that I would like to activate, is a PUK required for every new sim?

Sometimes with Samsung phones, it "locks" to previous mobile phone company. You can wait for moderators, but it will be quicker to contact Samsung directly to ask for PUK code. Either phone or do online live chat with Samsung. This has happened to previous Samsung owners in the past.

Tried it on 2 different phones, a galaxy S7 and a J3. Both phones are unlocked.
Is there a reason why a PUK would be required?

PUK is never required with a new PM SIM, unless it is inadvertently locked.
Try the SIM in a different phone.
What is make/model of your phone?
Reboot with the SIM installed and report the message.
