How do i request a refund. I ordered the sim card Oct 23 and have not received it yet and my account still shows Step 3: Review and Pay

I'm sorry you still haven't received your sim card. Unfortunately PM is terrible about sending sim cards out in reasonable amount of time. It's almost as if they don't want to onboard new customers.
If you're willing to give it another go, I would suggest purchasing a sim from Amazon for $5. It's eligible for Prime shipping. Alternatively Telus and Koodo corporate stores often carry stock for $10. Once you've secured a sim you can contact @CS_Agent via private message to request a refund for your initial sim card.

you can also ask agent to let you buy the sim card from Telus or Koodo store and reimburse you the cost

SIM cards can take up to 3 weeks to arrive.
If it doesn’t arrive by November 17 you can contact a CS agent.
