A few years ago, I received this holiday promotion:

$0 Free Holiday Giveaway: 400min INTL Long Distance (incl CA & US)

Can I use these minutes while I am in the US (to call either Canada or the US)? Or will I need an add-on package for calls?

No, they can be originating from canada to overseas only.

That's right you have to be in Canada to use that giveaway.It will not work in USA .you have to buy US Roaming add ONS.I hope this helps.

No, you cannot use:

"$0 Free Holiday Giveaway: 400min INTL Long Distance (incl CA & US)" 

while you are in the USA.

You need a Roaming Addon to use services in the USA.

"$0 Free Holiday Giveaway: 400min INTL Long Distance (incl CA & US)"  is for use only while YOU (your Public Mobile device) are in Canada.

@andrea302   These Long Distance add-ons are for calling from Canada to other countries

If you are in US and you need call service, or data  as well, you will have to buy Roaming Add-on

But consider using VoIP app like TextNow or Fongo to save some money.  They are free to call back Canada , as long as you have Wifi or Mobile Data

How To Purchase Add-Ons Using Your Self-Serve Account

To purchase Add-Ons using your Self-Serve account, follow these steps:

1. Click on ‘Purchase Add- Ons’ on the Overview tab and scroll down to the Add-On calculator.
2. Click the plus sign (+) to add and the minus sign (-) to remove selected Add-ons.
3. When you’ve added all your desired Add-Ons, click ‘Buy Add-Ons’ or ‘Make a Payment’ below the calculator:
   • You will see ‘Make a Payment’ instead of ‘Buy Add-Ons’ if there are not enough funds in your account to make the purchase.
   • If you don’t have sufficient funds to purchase the Add-Ons, you will be asked to make a one-time payment equal to the price of the Add-On, less your current available funds. Click ‘Submit’, then ‘Go back’: now the button below the calculator will say ‘Buy Add-Ons’.
4. Select ‘Confirm my purchase’, and you’re off to the races!

5. after you Add-on you have to do Rebooting your phone

   what is mean for Rebooting the phone means to turn off your phone and turn it back on again.
