Did you purchase this addon before you travelled to the U.S.? If so, how soon before?
If you completed the 2-step process for buying addons, the Roaming Addons start as soon as you add them (no activation needed) to your account/overview.

Did you check if the add-ons show up in My Plan of your account?
If they don't and Available funds show the money you loaded to your account sitting there, you've missed the second part of the add-ons purchase. So go back to the Add-ons page to complete the purchase.
If the add-ons are indeed present and you've verified all necessary settings to make roaming work but still don't succeed, you'll need to contact CS for help.

When you are in the US, you have to enable roaming and for data make sure you have the correct APN settings.

Getting that add-on is a 2 step process. First, add $$ to Available Funds, finally, go back and "buy" the add-on you want. Restart phone. Might have to manually select either of T-Mobile or AT&T. The add-on only works while in US.
edit; Are you still in US ? If not, go to top of page and click Get Help / Chat with Simon to start process of CSA assistance to have your $$ credited back to your account...not refunded.
Watch the envelope icon on top right side of page will be highlighted when they respond.
