No access to phone at moment so can't receive otp code. No option to send to email. Account in good standing.

Call 1855-4PUBLIC, enter your phone number if that system cannot find your account, then yes , the account is permanently closed

If you've closed an account, the SIM is deactivated. If the account has gone more than 90 days, the SIM is deactivated. If it's been less than 90 days, the SIM may still be active. There really is no test to confirm if a SIM is defective or not. If you feel it is defective, SIM cards are cheap. You can always get a new one if you want.
To get a new SIM card the fastest routes are these.
https://www.publicmobile.ca/en/bc/pick-up-SIM - Find a local store in your area.
Amazon - Here you can get it cheaper and maybe a few days wait $4.99
Or find a Mobile Klinik near you. https://mobileklinik.ca/stores/

What makes you think your sim might be defective ? The sim is a robust item and should last for a long time within your cell.

If your account is activated, stick your SIM into phone and see if you get service.
If it does NOT work in one phone (phone might be defective) try another phone.
If SIM card has been used before and service was Cancelled then that SIM cannot be re-activated again and you can recycle it.
