SIM disabled. Need PUK code

Phone is giving me message that the SIM (physical) is disabled and need the PUK code.

PUK does not appear on the plastic credit card that the SIM came with.

Help!

Current customer. phone is showing "SIM card is PUK locked - Emergency calls only"

More details would be helpful.  New customer versus existing? 

What exact message is your phone showing?  

If new customer, is your phone locked to previous carrier?
