It can be left blank. You only need to fill in one of those fields as per numberr portability guidelines.
These are actually questions to be asking your new carrier (I'm assuming you're leaving Public Mobile since you mentioned porting out). The new carrier is resposible for providing all support and troubleshooting related to number porting.

They don't actually need it. The porting rules specify that only one of account number, account PIN, or IMEI is needed. Using more than 1 actually increases the chance of porting failure.

You don't need a pin for porting out. The other provider sends a request to public mobile and public mobile sends you a text message asking your permission. You say yes, the number goes to the other provider. Please make sure your account is active and not closed or the port won't work.
