Started this process last night-still waiting for a response.
I provided all the requested information, including the SIM card number.
This morning I had to signup for a new account since I could no longer access my existing account. (Existing account was nvmos@teksavvy.com, my current account is davegirard@teksavvy.com).
I really need that code to reactivate my SIM card to allow me to contact Apple for assistance on another issue I encountered recently.

Please clarify your request.
You need a sim unlock code or your account was canceled and you want to restart the service again? Sim cards once canceled cannot be reactivated. A new on will need to be purchased.
