Hello there,
My mom is coming to visit me next week from another country. She would bring her own phone but she would like to have a Canadian number to access data while she's in TO for a month.
Would you let me know if I could activate your SIM card and just use it for a month?
Can I save the card for her next visit probably in another year? Do I get charged extra to keep the card activated?
Thanks in advance!

Or you can switch to a 10-day texting only plan ($11) after your mom leaves, and make sure you make payment every 99 days (10 days for the plan + 89 days before the card is disabled). So to keep the SIM card alive for one year you'd have to pay more than 4 times $11.

Phone:
If your mom's phone is unlocked, yes you can use it. But, make sure whether the phone is not country locked and whether band width is compatible in Canada.
SIM:
No. Once it gets deactivated after 90 days, the SIM is not usable anymore. You will need to purchase another SIM next time, unfortunately.. 😞
