Last time I traveled Canada for work I purchased a phone 10/2018, do I need a new SIM card or phone and SIM card?

If you have the same phone it will still be compatible pm. If it is VoLTE compatible and you activate a new Sim card/account it will be enabled for VoLTE if it's on the approved VoLTE list (YMMV if it's not listed but VoLTE capable).
Unless you maintained your old account the purchase of a new Sim card and activation once recieved is necessary. Currently pm Sim cards are $5 on the pm website store but shipping can take between 5 to 10 business days or longer....? Or not at all sometimes? They are also available online at Amazon.ca for various prices or Canadian Cell Supplies for $5 with free shipping in 5 business days across Canada or pick up in the GTA. They can be purchased in person for $10 at corporate telus and koodo stores or some mobile klinik stores. (Call ahead to see if they are in stock.)
A valid Canadian or US credit card or credit/debit card is needed for activation. It will be automatically registered for autopay. Vouchers can only be used for activation via customer support and are preferably used for payment use only.
Don't forget to use a referral code for your $10 bonus referral credit. If you don't have a friend or family member with one you may choose a community member to private message and strike up a friendship and ask for their code. They will be happy to oblige. Unsolicited referral codes are against the rules and they should not be used but rather reported. But that is up to to your own discretion.

A device that worked on PUblic Mobile while you were still here would still work. As for the SIM card, that depends on whether the Public Mobile account that you opened previously was kept open/active.

You need a new SIM card, yes. (EDIT: I’m presuming the account was inactive for greater than 90 days in stating prior comment)
The device you used then should work again.
