Have you used this phone on Public Mobile network in the past ?
If it's a new (used) phone...make sure it's unlocked and not reported as Stolen or Blacklisted.

Lots of threads lately have discussed this. You'd need to contact an agent to request that PUK (or PIN unlock code). Here's that link again: https://productioncommunity.publicmobile.ca/t5/notes/composepage/note-to-user-id/22437

...for your phone? Or for the SIM? Is your phone unlocked?

Do you need a new voicemail PIN? or a new PIN to access 611?
For voicemail, login to selfserve.publicmobile.ca, and on the overview page click "Reset Voicemail Password".
For 611 PIN, you'll need to open a ticket with the CS agents to change it if you don't remember what you entered when you activated your account.
The chatbot/automated ticket creation system is a little finicky the last week or so, so you might fare better by sending a message directly to the CS agents using this link:

Good luck!
