The USA has numerous global territories outside their contiguous mainland. Which sometimes come and go, as it were, in long-distance calling package terms. A USA long-distance add-on can always call the USA ... it's just the specific locations, certain remote islands, etc, may or may not be considered USA territory as time passes.
Hong Kong vs China and Ireland vs UK have had different availabilities in the "international" long-distance add-on packages. A little bit of a mess until Public Mobile finally created separate long-distance add-ons.
Some European countries would sometimes quietly come and go before. Nobody would even notice until somebody complained about suddenly no longer being able to dial the country. Ukraine is conspicuously absent these days, since their telecommunications infrastructure is obviously not a priority for them right now.

Not all countries are covered by ad-on or holiday bonuses.
