About your account, contact the moderators by creating a ticket here:

Type in "rep", then select [Contact Us] and then select [Submit a ticket / Click me!]
Follow the instructions to fill in and submit your ticket.
For the discount, you will get the first credit within 30 days. Your second discount and onwards will appear before your plan renewal date, every 30 days.

As stated above, more details about your situation is needed. Is your phone working? Are you able to access self service account.
You are likely talking about the Winback offer of $15 per month x 12 months. According to terms and conditions from your email, you will get the first credit within 30 days and then get monthly credit afterwards. You should receive a SMS text when you get the credit. It does not look like that you will receive an initial confirmation email or text for the discount.
