To submit a ticket to the Moderator Team, please start a conversation with the Public's virtual assistant, SIMon by clicking here or the icon (bottom right of your screen).

How long has your account been Suspended ? If it's been over 90 days since the last time it Expired, then your account has been closed for good. You'd need a new sim and start a new account.
If your account only recently expired and you still can't add the voucher...you should get ahold of a moderator as explained above.

Perhaps not all Petro cans use the same system for their vouchers. You can always call the station you purchased it at and ask what is the norm...double check the numbers again. If it's urgent you can contact the moderators to add them for you....they don't appear to be too busy today that they will probably have a fast response time.

Is there two sets of 12 digit numbers on your voucher? One us the serial number....the other is the pin number. Try the the other number. Using 611 can be easier than self serve.
Edit: Info was accessed via the "Voucher Catalogue" found by the link supplied or typing it in the search bar. Please feel free to add examples of your vouchers once loaded on you account. You can thank @hyT for that info.......

Did you try on both the 611 service and the self-serve? Are there any other 12 digit codes on the receipt?

Sometimes it takes up to 24 hours for the voucher to become active. When did you purchase it?
Are you adding the voucher by self service or dialing 611. If you are using self service make sure you clear your history/cache and then open a new browser in incognito/private mode.
