Form for a Ticket is not working

I need to submit for a ticket to return a certified cell phone within 30 days yet the form in Community will not let me submit? Need help please.

HI @rolande-1945   What was the trouble you get?  Unable to open ticket?

 

If so, message PM support directly if you cannot open via Chatbot

After ticket is submitted, make sure to check your Community Inbox(top right corner envelope icon) periodically for response from PM's CS Agent

@Chuckwagon The link lets you private message Public Mobile workers directly. Keep an eye out on your inbox on these forums. The envelope icon top rightish is your inbox. That's where they will write you back.
