is there a proper form to send email to agent to cancel credit card info and disconnect service?

No there isn't. You have to cancel your account yourself.
If you don't want your phone number anymore, sign into My Account Account and turned the subscribe to off. After 90 days of non payment aacount auto close and number lost.
If you want your number, keep account active and find a new provider to port out to then number auto close on PM.
Once that is completed, if you want your credit info removed from file then you submit a request here.

If you want to cancel your account with PM you can port your number out and your account will automatically close. You can also go into self service and remove autopay. After 90 days from your next renewal date without a payment your account will be closed automatically.
