Out of nowhere, my phone lost service and I got a notification that my phone wasn't activated. I tried resetting/formatting/etc. Then I checked the IMEI and it said the phone has been blacklisted. I bought this phone brand new 4 years ago and have never had any problems with it. How did it get blacklisted? I'm the only owner of this phone. How can I solve this?

Contact Moderator_Team and Apple to find out who blacklisted. Then send proof receipt to show ownership. If carrier doesn't cooperate, file complaint with CCTS. Yes, it was probably a mistake or someone cloned IMEI to get another device and could have stopped paying bill. They not supposed do that but some carriers do.

Could be human error / fat finger etc.. or problems with your previous provider, since Public Mobile doesn't sell phones. It has to be someone who has your IMEI.
I know the Lost / Found option in the menu can't trigger this. Perhaps you can contact the Moderator.

Dang. Why would this have happened? I don't understand what could've led to this.

@mattmacneilOnce it is in the database I am not sure you can get it out.. I would suggest take it to the Apple Store and see if they would do a goodwill and swap you with a clean one.. do bring your proof of purchase etc. Only providers can add devices to the database.
I think about it like the phone version of the no fly list.. there are so many databases connected to this.. and syncing could always be an issue. I won't trust the IMEI won't appear on the list in the future again even if you get your phone off it once.
