What information is required for proper authentication to make a request with customer service ?

Not really, after you contact them and ask your question (if you have any) they'll ask for verification information (if needed).
I don't quite understand why you need this information exactly before you contact them. It's like if you contact a bank for your account details they'll ask info to verify like your birth-date, your mothers maiden name etc. You cannot know exactly what they'll ask prior as that would for one impose a security risk.

The link you provided opens a customer service request. they must have a standard procedure as to what is required to authenticate a user. do they have a document ?

Well, this depends on the situation and if you have a question feel free to contact the service agent at, https://productioncommunity.publicmobile.ca/t5/notes/composepage/note-to-user-id/22437. While we don't know exactly what they will ask, it's basically all the information that you'll have in your account. If you are talking to them on behalf of another user make sure you let them know about it as well so as to avoid any confusion.
