If the third code doesn't work, the only other option I can think of is to get a new SIM card. Then get Public Mobile to reimburse you for it.
https://www.publicmobile.ca/en/bc/pick-up-SIM - Find a local store in your area.
Amazon - Here you can get it cheaper and maybe a few days wait $4.99
Or find a Mobile Klinik near you. https://mobileklinik.ca/stores/

As far as I know, iPhones don't require "PUK" codes. Public Mobile should be offering you a PUK code for your locked SIM. Does the screen look like this?

It’s an iPhone purchased directly from Apple.

What brand is the phone and What was the exact error?
If the message is "SIM network unlock PIN" or "SIM ME lock code NP" or "SIM not Supported" or on an iPhone's About->Carrier Lock showing "SIM locked"
You have to ask the carrier where you got the phone from for the carrier help unlocking it.
(Sorry, for this particular issue, PM support would not be able to help)

Public mobile support is telling me that I need to get the PUK code from the phone manufacturer. 😳
