is that a sim card PUK You are looking for? if it is , PM support can help

but if the sim card can work on another phone and just asking for PUK on a particular phone, it is a carrier lock problem and you have to ask the unlock PUK from where you got the phone from originally

submit ticket with support . using the chat bubble bottom right of screen
