If you are needing to pause your account for longer than 90 days but want to keep your phone number, you can look at an option like Numberbarn. You can store your number there until you're ready to port it back to your subscriber.

Short answer is no.
You will need to re-activate your plan before the 90 days of suspension is up. Pay for 30 days and then pause account for another 90 days.
If you account is suspended more than 90 days, your account will be permanently closed and phone number lost.

No, if longer than 90 days. Your account will be closed, SIM will no longer work and your number will be returned to the carrier you got the number from.
