Further to the above, if you have a Samsung phone, they have a method to get codes too.

Did you try to unlock your phone unsuccessfully several times and now need the PUK code? If so, you will need to contact moderator.

If your phone is locked to your previous carrier, you will need to contact them to unlock your phone. It is a free service now.
