It states "Enter PIN to Unlock" .When I enter my pin it states PIN Unlock attempt unsuccessful. What do I do?

As a test only, does your sim work in another phone? An unlocked or Telus locked phone would be good to try.

@thomasgriese when you use SIMon Bot by clicking on the bottom, right chat bubble, and type in "Enter PIN to Unlock", you get the below results.
Does following through to one of them get you to where you need to be?

By any chance have you recently acquired this phone?
Hopefully it was from a business or friend who can assist with identifying the prior Wireless Service Provider.
They'll need to unlock the device for use on the public mobile network.
While you're at it you may as well make sure the device is not reported as lost or stolen. Check here:

@thomasgriese : You need to contact the previous (Canadian) provider and have them give you the unlock code. If it's a Samsung then they can help you too.
