You'll need help from a custumer support agent to open a self serve account IF you activated in-store.
That is unless you mean you haven't yet activated the SIM card, in which case you click activate at the top of the page.
To contact a Customer Support Agent, there are 2 methods:
Watch for their response in your Community private mailbox which will be indicated by a number on the small envelope icon to the left of your Community avatar.

When did you activated your PM account as PM had switched to online activation only for a while?
If you activated your account in-store recently, which store?
To create your self-serve account manually, go to login page and click the Register Now button.
