Either my username doesn't work or my password. After two tries, I get message I've exceeded number of tries. I've tried different combinations without success.
I need a moderator to help me get in and reset. Or what else can I do?

I think that the person is locked out of self serve because of the title but pannobaji could be talking about unlocking a phone.

You can't recactivate your phone until you get your self serve password working.

Unfortunately, there is no customer service via telephone support at PM. The SIMon chatbot will submit your ticket to moderators and give you a ref. number for your issue. Hearing back from moderators can take anywhere from 2-48 hours/or longer, depending upon the number of requests ahead of you.

Try the forgot password link. Also try using a different browser and incognito mode. Clear your cache and cookies then reboot. If you still have problems then submit a ticket by clicking on the question mark on the bottom right.
