I need a Written confirmation from the cellular company of date and time you notified your wireless service provider of loss or theft. How can I obtain this?

I also assumed you login to your account and clicked on Report Lost/Stolen Device in the Account tab. It will probably have a date indicated.
Otherwise, contact a CS_Agent.

I assume you used the lost or stolen facility in your account? Perhaps you took a screenshot for proof, or is that not adequate?
