To pre-verify your account include your full name, address, email, phone # and 4 digit acct pin #.

Another is to login to your Self Serve account and print out your Payment with just the name of the registered to the account and dates.

You can contact a CS_Agent for a bill payment but ask if it will include the address.

@danielkua You'll need to contact support for that. They'll send you a PDF. Due note people that require this usually ask for it quarterly or yearly.  Not sure if it will show name and address as PM is prepaid and does not do a credit check. You can have any name or Addy entered as it does not mean much with PM.
