I had same issue ,  your first name,  last name field should be 11 chrs or less then try. sit should work.

Thans a lot it is working after the data APN  setting done in the phone

@Shazia_K, @Mary_M, and @Simon_O @MarieHelen_L Actually I went through two step process ,Add-on section shows both my data and long distance services are added.Still my data is not working but long distance is working fine. Any idea?.
