as discussed in this thread the port form has been improved.
as suggested by @srlawren I am posting some additional changes.
1) recommend that the account number be mandatory and remove the other two or use the IMEI/MEID and pin as an optional back up. ie pin is a known fail with chatr (according to some)
2) add (No spaces, No dashes) to old account number description
3) tell users to make sure the IMEI/MEID on their phone matches the IMEI/MEID on their account. (if phone was changed without old providers knowledge, these numbers will be different)
4) change name field to say (name EXACTLY as shown on old provider account, like what they say with credit cards)
