At the end of the form, I tried to hit the “Continue “ button but it doesn’t go through.

Clear your cache/cookies, use another browser in incognito mode.  You may need to wait one hour before retrying too.

@Toantran8856  Insert your card in your phone does it connect to Public's network? If yes, you are good to go. Access your selfserve account and make sure your plan details are correct.

 

If not, wait an hour and try the activation again.
