This post tells where the toggle is. It's what I was suggesting about that it might be toggled to not play the caller ID. Yes, by default it would.
But folks on limited minute plans might not want to hear everything the voicemail wants to say. And then maybe sometime later want to and forget that they set it.
And for the thousand words version:

Sometimes, people don't always check for missed calls and messages right away. If that's the case, there wouldn't be any other way to know which missed calls in the call log lead to a message being left other than to listen to the messages. Also, the call log won't record phone numbers if the phone didn't ring. Fortunately, the phone number is provided automatically when listenning to the message.

As long as the number wasn't blocked by the caller, the phone number automatically reads out by default when you play back the message.

The caller will have had to have exposed their number. Then you might want to check your voicemail options and turn on verbose mode.
A couple thoughts.
