How far have you gotten with trying to get into your account? Have you waited the sometimes 40 seconds it can take for the self-serve page to load?
Is it that you forgot your Username (which is your e-mail that you registered) or the Password (which is what you setup when you first set up self-serve)?
If forgot Username: Search through e-mail accounts for activation confirmation. If cannot locate, contact moderators as @computergeek541 provides.
If forgot Password (and you know Username): Use the forgot password option.

Unfortunately, there isn't any way to change the credit card on file without first getting into your account. You can first try the forgot password option at: https://selfserve.publicmobile.ca/forgot-password/
If that doesn't work, you'll need to open a ticket by going to https://widget.telus.tiia.ai/publicmobile/publicmobile.html
After opening that link, type in something such as "password" to get to the right place.
In the meanwhile, and if your service has been cut off, you could purchase a voucher from a store and apply the voucher using *611. In addition, you call purchase a voucher from a website such as ding.com or recharge.com.
