He went into his account to see if has rewards to use. When he logged in it said his account was unsubscribed. Why would it change? He has not been in his account for some time and his credit card is on file. Has this happened to anyone else?
The concerning part is his account renews soon and if he didn't log in his account would have been suspended for non-payment.

Maybe a cache problem. Ask him to login again using Incognito mode

Try to use computer web browser. Clear cache; use incognito mode. PM has caching issue so that might be an issue.
As long as he has service you should not worry. Just check if bill is paid on time (as autopay can fail). Confirm switch is ON for Subsribed:
