Why is my daughter's phone asking for a PUK code?

Ok great.  Some poelpe get a message back with the hour. Some indicate days due to delays, just have to hang tight...

It says a PUK code is required to unlock.  She cannot send or receive texts/calls.  I've submitted a ticket.
