I bought a US add on and it didn't work.  How do i get the refund?

@shaji are you still in US?  you tried connect to AT&T or T-Mobile?  make sure you change from Automatic to one of that 2 carriers

 

Did you buy Data and/or Voice/Text?  If you have data, make sure you enable Data Roaming on the phone

 

If yiubare back in Canada already or just want a refund,  open ticket with PM, they night be able to reverse the transaction and put the fund back to your account for future use

Do you still see the balance in your account? Maybe you didn't actually complete the purchase. Otherwise, with all the network changes going on in the US, things may not have worked anyway without some adjustments.
